How To Join?
Applying for membership is easy, simply print out and complete the Membership Application Form and then visit one of our offices. Make sure you have the items listed below to complete the account opening process.
Copy of driver's license or government issued ID
Second Form of ID (social security card or Hospital I.D. Badge)
Proof of Membership Eligibility (Hospital I.D. Badge/ or name of family member that currently holds an account)
Minimum opening balance of $25.00 for your Savings account
